The forms listed below were designed to function in either a Microsoft Exchange Server environment or independently on the Microsoft Outlook client.  These samples were customized and are currently being used by one of our clients.  They can be programmed automatically to be sent to a specific recipient, or you  can simply choose one from your address book.

To view any of our Custom Forms, simply click one of the links below Outlook Custom Forms.

Note:  You must be running Microsoft Outlook in order to view the forms. Also, be sure to select "Enable Macros" when prompted in order to activate the VBScript.

If you would like to see how the VBScript behind the form works, you will have to download these Microsoft Word templates.

After selecting the Word template you would like to download, simply right click the file to be downloaded & select "Save Target As".  You will need to change the file extension from "doc" to "dot", but do not modify the file name.  Make sure that you save this file to your C:\temp directory.

*Be sure to scroll to the bottom of each form & click the "Print this Form" button in order to print using the code built into the forms. This will open the Word template file, transfer all data from the custom form & print of the results.

For some versions of Outlook, you cannot open the custom form directly from the website. Instead the form needs to be published in Outlook in order for the code to run. Open the custom form & from the menu bar select tools, forms, publish form as, publish form into the Personal Forms Library. To open the form from Outlook, select tools, forms, choose form, select the Personal Forms Library from the drop down list, & choose the form you saved.

Note:  In order for the script to work correctly, the file name must not be changed (other than the extension), and it must be saved to C:\temp.  You must also be running Microsoft Word.

The above forms, are just a sample of what can be done using your existing software.
